To receive the scholarship this form must be submitted again before the stated Office of Distance Education deadline and must include a list of the duties performed.

| Read the statements below before submitting your information.
As a cooperating student you may be expected to operate the videoconference
equipment, arrive 15 to 30 minutes early to ready the room and/or equipment,
receive and distribute class handouts at your site, and help troubleshoot the
equipment in case of technical problems. Notify JSU immediately of any equipment failures or malfunctions. You may contact us via email at mcamp@jsu.edu or by phone during class time (256) 782-5033 or during the day 8:00 a.m. to 4:30 p.m. central time (256) 782-8127.
Note: You will be trained in the use of any equipment which you are expected
to operate, and technical assistance by phone will be available in case of
technical difficulties.
Warning: You may NOT be the cooperating student for a videoconference
SITE for which you are not registered.
